Measurement of 11-deoxycortisol levels (11-DOC, Compound S) is used in the diagnosis of adrenal cancer and congenital adrenal hyperplasia.

The measurement of 11-deoxycortisol after metyrapone administration is used in the diagnosis of adrenal insufficiency and Cushing's disease.

Cortisol is synthesized from two successive hydroxylations of 17a-hydroxyprogesterone. The first hydroxylation results in the formation of 11-deoxycortisol, which is then hydroxylated by 11-b-hydroxylase to form cortisol. Metyrapone administration prevents the conversion of 11-deoxycortisol to cortisol, thereby stimulating the adrenal glands to produce more 11-deoxycortisol. Decreased response after metyrapone administration occurs in patients with adrenal or pituitary dysfunction and in some patients with hypothalamic diseases. Patients with myxedema, sometimes in pregnant women and those taking oral contraceptives, may also have a reduced response to metyrapone. 11-deoxycortisol has no glucocorticoid activity.

High levels of 11-deoxycortisol are found in adrenal cortex tumors. Taking glucocorticoids (hydrocortisone, dexamethasone, prednisone) increases 11-deoxycortisol levels.

Low levels of 11-deoxycortisol are found in amyloidosis and in patients with post-traumatic stress disorder.
