Levetiracetam (Keppra)

Serum levitiracetam is used to monitor drug levels in the body during treatment, to monitor patient compliance with treatment, and to evaluate any drug toxicity.

Levitiracetam (Levetiracetam, Keppra) is used in the treatment of myoclonic and tonic-clonic seizures and also (except where appropriate) in manic conditions and in migraine prevention. Levitiracetam has good bioavailability and rapid steady state. Its hepatic metabolism is minimal and non-oxidative, making it safe for co-administration with inducers or inhibitors of liver enzymes. The major metabolite of the drug is a carboxylic acid derivative, which is inactive and represents about one quarter of the administered dose. Levitiracetam is excreted by the kidneys and has an average half-life of 7 hours in adults. Renal impairment warrants therapeutic monitoring of serum drug concentration and dose adjustment.

The toxic effects associated with the use of levetiracetam include reduced hematocrit and erythrocyte count, decreased neutrophil count, drowsiness, weakness and dizziness. These toxicities can also occur in drug concentrations in the blood within the therapeutic range.
