17-Ketosteroids, Urine

Measurement of 17-ketosteroids in the urine is used to test adrenal cortex function.

17-Ketosteroids (17-KS) are substances that are formed when the body metabolizes the steroid hormones produced by the adrenals, testes, and ovaries. Adrenal androgens measured as 17-ketosteroids include dehydroepiandrosterone, androstenedione, etiocholanolone, androsterone, and their metabolites. In males, about two-thirds of 17-ketosteroids are secreted by the adrenal cortex, while in females almost the entire amount of 17-ketosteroids is secreted by the adrenal cortex. Thus, for both sexes, the measurement of 17-keto steroids in the urine reflects the function of the adrenal cortex.

Interpretation of Pathological Index
  • Increase: Cushing's syndrome, early puberty due to adrenal hyperplasia, normal stress (surgical procedures, burns, infections), stimulation by ACTH or gonadotropin
  • Decrease: Addison's disease, ACTH deficiency, medication (corticosteroids, estrogens, oral contraceptives
