Amiodarone, Serum

Serum amiodarone measurement is used to monitor the levels of the drug in the body during treatment.

Amiodarone (Angoron) is a fat-soluble, class III antiarrhythmic drug, with several mechanisms of action, including weak negative inotropic action in combination with compensatory vasodilatation, prolongation of the refractory period of cardiac tissue, suppression of sinoatrial automation, and slowing the impulse conduction of the atrioventricular node. It is used to treat patients with a history of very severe arrhythmias that cannot be controlled with other drugs and after myocardial infarction for symptomatic or prolonged abdominal arrhythmias.

Because amiodarone is fat-soluble with a long half-life, it may take up to 4 weeks for steady-state levels to be achieved, with the drug remaining in the fat storage sites for a long time after discontinuation. Amiodarone is metabolized and excreted mainly by the liver.

The potentially life-threatening side effects of amiodarone (acute hepatitis, pulmonary toxicity, obstructive bronchiolitis, pulmonary fibrosis) require close monitoring of blood levels as well as clear instructions to patients regarding side effects.
