Flunitrazepam, Serum

The measurement of serum flunitrazepam is used to monitor drug levels in the body during treatment, to monitor patients' compliance with treatment, and to evaluate any drug toxicity.

Flunitrazepam (Flunitrazepam, Hipnosedeon, Ilman, Neo Nifalium, Vulbegal, Narcozep, Rohypnol, Primun, Rohipnol, Roipnol) is an intermediate-acting benzodiazepine that is used as a hypnotic, sedative, anxiolytic and muscle relaxant drug. In general, flunitrazepam as a hypnotic is intended for the short-term treatment of chronic or severe insomnia that does not respond to other hypnotics, especially in hospitalized patients.

Oral administration of flunitrazepam results in an 80% absorption rate, whilst the bioavailability of the drug as a suppository is approximately 50%. Flunitrazepam has a half-life of 18-26 hours and an active metabolite with a half-life of 36-200 hours, which means that the effects of the drug after night administration may persist into the next day. This can affect the ability of users to drive safely and increase the risk of falls. Flunitrazepam is lipophilic and metabolized in the liver through oxidative pathways. The enzyme CYP3A4 is the major enzyme for the metabolism of the drug in liver microsomes.

Overdose of flunitrazepam can lead to excessive depression, balance and speech disorders. This can develop into respiratory depression or coma and possibly death. The risk of overdose is increased if taken in combination with CNS suppressants, such as alcohol and opioids.
