The measurement of hydroxyproline in urine is used for the laboratory diagnosis and monitoring of Paget's disease of bone, for investigating inflammatory or neoplastic bone involvement, as an indicator of activity in acromegaly, for monitoring growth hormone therapy, and as a complementary test for the diagnosis of Marfan syndrome.

Hydroxyproline is an amino acid found in collagen. Urine hydroxyproline excretion is a helpful indicator of collagen metabolism (degradation). It reflects the rate of bone breakdown and is a good test for monitoring the status of Paget's disease.
Possible Interpretations of Pathological Values
- Increase: Acromegaly, Albright syndrome, bone cancer, bone metastases, osteopenia, burns, congenital hypophosphatemia, diabetes mellitus, gastroenteritis, growth, fracture healing, hyperparathyroidism, hyperpituitarism, hyperprolinemia type II, hyperthyroidism, Marfan syndrome, multiple myeloma, osteomalacia, osteomyelitis, Paget's disease of bone, psoriasis, rickets. Medications: Growth hormone, parathyroid hormone, phenobarbital, sulfonylureas.
- Decrease: Hypoparathyroidism, hypopituitarism, hypothyroidism, malnutrition, muscular dystrophy. Medications: Acetylsalicylic acid, antineoplastic agents, ascorbic acid, calcitonin, calcium gluconate, corticosteroids, bisphosphonates, ergocalciferol, estradiol, estriol, glucocorticoids, mithramycin, propranolol
